Reception & Patient Service Department

When you arrive at Apollonion Private Hospital, go to the Reception & Patient Service Department to register your personal details.

The Reception & Patient Services staff will help you fill out all the necessary forms, walk you through the admission process, and then you'll be taken to your room to settle in.

Introduction

DON'T FORGET! WHEN CHECKING IN MAKE SURE YOU HAVE WITH YOU:

Please bring with you:

  • Your ID
  • Your doctor's referral (GeSY).

Before you are treated with us, we recommend that you have with you:

  • Personal hygiene items: toothbrush & toothpaste, sponge, comb, or hairbrush
  • Pajamas/nightgowns
  • Robe and slippers
  • Myopia glasses, contact lenses, hearing aids, dentures (when not in use, be sure to keep them in their protective cases to prevent loss or damage)

We suggest you leave your valuables at home.

Please do not leave personal valuables in the room during your stay.

The Hospital is not responsible for any loss of your personal belongings.

Pre-operative assessment

If you will have a scheduled surgery, your attending physician will inform you when you will have the pre-operative assessment.

Medication

Take with you or make sure you know the medicines you are taking so that they are taken into account for your treatment. They will be returned to you when you are discharged.

Also, tell your doctor about over-the-counter medications (generic, homeopathic, herbal, etc.) that you have taken in the last trimester, whether you continue to take them or have stopped taking them.

Medical history

Carry a copy of any medical information related to your hospitalization such as x-rays, diagnoses, test results.
